Jane App: Appointment scheduling, intake forms, clinical notes, invoicing, and video sessions are managed through Jane App. Jane App is compliant with PHIPA and stores data on Canadian servers. You can review Jane App's privacy policy here.

Zoom: In the event that Jane App's video platform is temporarily unavailable, sessions may be conducted via Zoom. Angela Mason uses the PHIPA-compliant version of Zoom for health care providers.

Google Analytics 4 and Google Search Console: This website uses Google Analytics 4 and Google Search Console to understand how visitors interact with the site. These tools collect anonymized, aggregated data such as pages visited, time spent on the site, and general geographic location. No personally identifying information is collected through these tools. Heidi Health: Session notes may be generated with the assistance of Heidi Health, an AI-powered clinical documentation tool. Heidi Health is used to support accurate and efficient note-taking during or after sessions. If you do not consent to the use of Heidi Health, notes will be taken manually, and your care will not be affected in any way. Everything you share in therapy is confidential. Your personal health information will not be disclosed to anyone outside of this practice without your written consent, except in the following circumstances as required by law and professional obligations:

• Where there is a risk of serious harm to yourself or others

• Where there is a reasonable suspicion of abuse or neglect of a child or vulnerable person

• Where disclosure is required by a court order or legal process

• Where required by the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for professional regulation or complaint purposes

In these situations Angela Mason will disclose only the minimum information required and, where possible, will discuss this with you beforehand.

Angela Mason Trauma Therapy takes the protection of your personal and personal health information seriously. Measures in place include:

• Clinical records stored securely within Jane App on Canadian PHIPA-compliant servers

• Password protection and two-factor authentication on all platforms used in this practice

• Use of PHIPA-compliant video platforms for virtual sessions

• Secure and encrypted email practices for client communication

Physical records, where applicable, are stored in a locked location accessible only to Angela Mason.

Personal health information is retained for a minimum of ten years from the date of last service, or until age 18 in the case of a minor, whichever is longer, in accordance with Ontario regulatory requirements. After this period, records are securely destroyed.

You have the right to:

• Access your personal health information held by this practice

• Request corrections to information that is inaccurate or incomplete

• Withdraw consent for the collection, use, or disclosure of your information, subject to legal and professional obligations
